Welcome to PurePath Functional Medicine. By accessing or using our website, services, or digital platform (including Practice Better), you agree to the following Terms of Service. Please read them carefully before engaging in any services.

1. Scope of Services

PurePath Functional Medicine provides wellness-focused, educational support services. Our offerings include:

  • Wellness consultations and check-ins

  • Lifestyle and nutrition education

  • Functional wellness lab education and general trend interpretation (ordered through a physician service such as Rupa Health)

  • Stress management and wellness strategy development

  • Referrals to licensed providers when appropriate

We do not diagnose, treat, or prescribe for any medical conditions. All medical care must be coordinated with your primary care provider.

2. Client Eligibility

Services are available to adults 18 years or older. By booking an appointment or accessing our platform, you confirm that you are legally able to consent to services and understand the non-clinical nature of offerings provided.

3. Client Responsibilities

You agree to:

  • Provide accurate and honest information during consultations and on intake forms

  • Acknowledge that all clinical concerns will be referred to a healthcare provider

  • Use the information provided for personal education and wellness purposes only

  • Maintain respectful communication with staff and within any group offerings or office hours

4. Appointments, Cancellations & Payments

Appointments are scheduled through our Practice Better portal. Clients are responsible for managing their own scheduling, and must provide at least 48 hours notice for cancellations or rescheduling.

Payments are due at the time of service or as outlined in your membership or package agreement. We reserve the right to suspend services for non-payment.

5. Confidentiality & Privacy

All personal and health-related information you share is handled in accordance with our Privacy Policy and HIPAA guidelines. We use secure, HIPAA-compliant platforms for communication and data storage.

6. Disclaimer of Medical Services

PurePath Functional Medicine is operated by a registered nurse providing wellness education and lifestyle guidance. We do not offer medical treatment, diagnosis, or prescriptions. Our services are not a substitute for care from a licensed medical provider. Always consult your physician or other qualified healthcare provider with any health concerns or before starting any new wellness program.

7. Intellectual Property

All content, materials, and resources shared through PurePath Functional Medicine are the property of the business and may not be copied, redistributed, or repurposed without express written permission.

8. Limitation of Liability

You agree that PurePath Functional Medicine and its staff shall not be liable for any direct, indirect, or incidental damages resulting from your use of the information or services provided. Services are offered “as is” and at your own discretion and risk.

9. Changes to These Terms

We may update these Terms of Service at any time. Updated terms will be posted to our website and effective immediately. Your continued use of our services constitutes agreement to the updated terms.
